Subscriptions for hormone replacement therapy are month to month. Patients may cancel at any time without penalty. 

 

Here at Swan City Wellness we acknowledge that while many patients will see the results they aim to achieve, not everyone may meet their intended weight loss goals and/or hormone related expectations. Due to the fact that most of these medications are ordered specifically for each individual patient, we are unable to provide refunds, returns or exchanges at this time.  

 

Patients must sign a consent form acknowledging this returns/refunds policy before moving forward with treatment

 

Medications are ordered/intended for patient use only. 

 

All fees for service are presented in US Dollars only.

 

Pt’s payment information is saved within the merchant payment processing system only.

 

Patient/Consumer Information is protected by Federal Law and the HIPAA Patient Privacy Act. Information submitted by patients is intended for provider/patient use only and will not be shared or sold to any other companies.
